Child's Social Security card, if available;
For non-Florida students, evidence of a health exam conducted within the last 12 months;
Florida Certificate of Immunization - immunizations may be administered by a private physician or at a clinic or one of the county's Health Department satellites (call 332-9601;
Proof of residence (lease agreement, mortgage acceptance letter, electric or water bill - see website for a complete list of what is acceptable);
The name and address of the child's former school;
Proof of custody if the child does not live with both biological parents.
